active template for your use. It should expand with your insertions. It is acceptable from a verified claimants email address.

Have fun and don't forget to fill everything in completely including assessments, interventions, full list of claimant injuries, soc and voc needs training etc etc.

Hey.....not scanned....as it came...just taken my details off.....you don't have to print this one until you have it all prepared....just use it on the pc....add to it, take away etc. until you get it right.

Thanks muchly Gloria. Last time I did my own they came back with "not acceptable as our format has changed" whewey.
