How to challenge a denial of ACC

If your claim for Accident Compensation has been denied, or if you’re unhappy with an aspect of the decision granting your claim, you can apply for a formal review of the decision. ACC must tell you, at the appropriate time, about your right to have a review

Glossary of ACC terms


ACC terms - A
A B C D E F G H I J K L M N O P Q R S T U V W X Y Z

ACC1
Request for assistance form. If a claimant needs extra help, such as further treatment, personal support or weekly compensation, they need to complete an ACC1 form.

ACC18
If a claimant is unable to work at their normal employment because of their injury, the Registered Medical Practitioner completes an ACC18, which describes how a claimant’s injury affects their capacity for work.

ACC32
Request for approval of payment for additional referred treatments. Completed by the treatment provider when initial treatments have been provided and the patient needs more treatment.

ACC40
Invoice for medical treatment services (bulk billing schedule). GPs complete the ACC40 to request payment from ACC for services provided to people who have been injured in an accident.

ACC41
Completed by a GP for referring a patient to another provider. Not to be used at the initial consultation.

ACC43
Used for specialists to apply to ACC for prior approval of hospital treatment costs. The specialist completes the form and sends it to the claimant’s ACC branch office for consideration before the surgery is done. An Assessment Report and Treatment Plan must be attached when the form is sent to ACC.

ACC45
First report of injury caused by accident – the form used to determine cover on a person’s claim. The form is held by treatment providers and completed by both the claimant and the treatment provider.

ACC054
Lump sum/Independence Allowance application form.

Ageing process injury
If the ageing process appears to be a significant contributory cause of a claimant’s condition, a medical report from either a GP or a specialist will be necessary to clarify the relationship between the injury and the effects of the ageing process.

A gimmick, no one but you can see your ip address, it turned up on a few forums in member signatures.



anyway take the time the download a full copy of the Judge Trapski report of 1994
read it then open a full copy of your files, claim details reports and all other reports, and compare.
