Old Lump Sum payments - info wanted

Hiya

After getting a copy of my claim files I found lump sum payments had been made but I didn't get them. I did a single claim (sensitive), had an assessment and other claims were lodged (don't know by who). My original injury file was "lost".

I'm looking for any info about internal ACC practices back then, especially questionable ones, and what I can ask for relating to the old system pre and post IIS (1992?) to try and clear this up.

Also when was file generated and up loaded into acc system (time date reason etc) Acc by law has 21 days to answer your official information request and you reason and give you all Acc information Acc holds about your claim
