To Whom it May Concern

I (name) request a full and complete copy of my file (client number)
between (eg. 01/01/2000 to 20/03/2011). Please include:

All computerised notes, hand written notes
All UCV II notes, emails (provides access logs. Note that around October 2011 UCV II was being replaced by another computer system)
All letters sent
All letters received, and applications made including documentation
All Trace notes (when changes are made the system records why they were made)
All Rate Audit Screens (Identifies how income support is broken down. Rate audit screens are used as a cross check between Trace notes
and UCV II are compared with Rate Audit to ensure integrity of the system. You can match information between with the Rate Audit screen
because Trace will show changes made, UCV II will show who was accessing the file at that time, and the Rate Audit screen will give details
on why the change occurred.

This request is made under the Privacy Act 2003.

(The Ministry has 20 working days to forfill the request)

IMHO and experience I believe you will be very lucky to receive most of what you are legally entitled to....

MSD/WINZ employ a Privacy Officer and one of their legal staff used to work for the Privacy Commissioner.

I believe dealing with them is like dealing with a bunch of slippery eels IMHO (in my honest opinion).

As for privacy well I do not believe they know the meaning of the word...
